Submitting an Order

Complete the "Sample Submission Form" and upload form via the order upload page.
Within several days we will send an e-mail confirmation that includes the "Agreement for Provision of Plant PCR Pathogen Testing Services" and a quote for the testing service.
If you wish to continue with the testing, print and sign the "Agreement for Provision of Plant PCR Pathogen Testing Services".
Include the completed "Agreement for Provision of Plant PCR Pathogen Testing Services" with your samples (see "Shipping Instructions"). We must have this to begin processing your samples.
